An expert consensus meeting was held to develop a framework for care of multimorbid patients that can be applied across Europe, within a project funded by the European Union; the Joint Action on Chronic Diseases and Promoting Healthy Ageing across the Life Cycle (JA-CHRODIS). Sixteen components across five domains were identified (Delivery of Care; Decision Support; Self Management Support; Information Systems and Technology; and Social and Community Resources). The description and aim of each component are described in these guidelines, along with a summary of key characteristics and relevance to multimorbid patients.

SOCIABLE supports personalized cognitive training interventions, designed by cognitive neuroscientists and health experts according to medically sound principles covering all cognitive skills.

Speech analysis for Hungtington or Parkinson (to detect possible signs of early impairment)

A model for digital neural impairment screening and self-assessment, which can evaluate cognitive and motor deficits for patients with symptoms of central nervous system (CNS) disorders, such as mild cognitive impairment (MCI), Parkinson's disease (PD), Huntington's disease (HD), or dementia. The data was collected with an Android mobile application that can track cognitive, hand tremor, energy expenditure, and speech features of subjects. We extracted 238 features as the model inputs using 16 tasks, 12 of them were based on a self-administered cognitive testing (SAGE) methodology and others used finger tapping and voice features acquired from the sensors of a smart mobile device (smartphone or tablet)

The SCIROCCO self-assessment tool is an online self- assessment tool with an objective to assess a region's readiness for integrated care. It builds on the conceptual Maturity Model for Integrated Care developed by the B3 Action Group on Integrated Care of the European Innovation Partnership on Active and Healthy Ageing.

VIREAS: Virtual Reality in Keeping Oldere Adults active

A set of virtual experiences VIREAS (The Set) has been developed based on several tests and studies among care home residents. The Set contains three virtual experiences: "Walk through a Forest," "Walk through the City Center," and "Travel." The interactive design combines computer graphics and 360?pictures. The Set is designed to positively influence older adults' self-expression and self-confidence, motivate them and stimulate their curiosity. The Set allows older users, especially those in care homes, to see places they cannot visit in the real world and brings them new stimuli they can share with others. The software VIREAS is beneficial mainly in care homes as tool for keeping residents active. The conceptual manual Virtual Reality and Its Use in Care Homes and Beyond focus both on the technical matters of virtual reality use and aspects of the virtual experience, together with the procedures that will make it possible to fully utilize the potential of this method.
