Description:

The SCIROCCO self-assessment tool is an online self- assessment tool with an objective to assess a region's readiness for integrated care. It builds on the conceptual Maturity Model for Integrated Care developed by the B3 Action Group on Integrated Care of the European Innovation Partnership on Active and Healthy Ageing.

Overview: 
After two years of intensive work, the SCIROCCO project has validated and tested the Maturity Model for Integrated Care that was developed by the B3 Action Group on Integrated Care of the European Innovation Partnership on Active and Healthy Ageing (EIPonAHA). The Model has been operationalised in the form of an online self-assessment tool. The Tool is ready to be used by stakeholders who are working on innovative solutions that support the delivery of integrated health and care. The tool has been tested in 40 regions across Australia, Europe, New Zealand, Singapore and the United States.

Initiatives: 
Many activities that need to be managed in order to deliver integrated care have been grouped into 12 dimensions, each of which addresses part of the overall effort. By considering each dimension, assessing the current situation, and allocating a measure of maturity within that domain, it is possible for a country or region to develop a radar diagram which reveals areas of strengths and also gaps in capability.

Relevance Description: 
Is a good as a resource available online for assessment of maturity level of the integrated care model, yet it should be noted that it is of limited relevance for small number of the interested stakeholders. Lot of resource and video tutorials available, stakeholders have the full support on how to use the self-assessment tool.
