Overview:
The present document provides the current status of the detailed description of the 23 Classes thatmake up the Classification of Integration Needs (CIN). It is a modelling tool that enables to represent ina systematic way the components of the innovative processes in the deployment of a Integrated CareProgramme and the related organisational changes about the tasks and the roles of each actor.
Objectives:
The CIN was conceived in a way that each Class of Needs denotes a particular component of care and cure services that can be addressed by an Integrated Care Programme, which in turn evokes a number of technological opportunities and a set of measurable properties that can be used to monitor the progress of the Programme during the transient period, when the final outcomes aren?t yet available. The present document suggests the default elements of the chain for each Class. These elements must be taken as a benchmark, which may be overtaken by the innovative Programmes.
Initiatives:
The intended direct users of the Classification are:national, regional and local policy makers; local procurers; clinicians;social workers; technologists; managers committed to local programmes.
